Why “detailed” and “accurate” aren’t just a claim

A lot of documentation shortcuts trade detail for speed. This doesn’t. Because the keyword list covers every relevant content area, the notes come out content-rich — they surpass what’s needed for a solid SOAP note, not the bare minimum.

And because nothing is generated or guessed, every word in the note came from a selection you actually made.
